How it works
TO+ is currently used by 15 danish municipalities and consists of a mobile and tablet app. The TO+ mobile app enables the home care assistants to register data, progress/fallback and changes in the physical and mental health of the citizen.
Data registrations are instantly accessible in the TO+ tablet app for overview and approval.
Home care administrative personnel can thus monitor daily progress, strengthen decision making and better prioritize resources and home care tasks.
It's possible to purchase up to 12 TO+ tools to add to the home care management platform.
